何正国:人类和结核病的冷战

在卫生部的疫情通报中,肺结核的发病率和死亡率每月都处在第一、第二的位置上。这个有着“痨病”之称的健康杀手,似乎一直没有停止对人类的侵害。华中农业大学蛋白质组学研究中心的何正国带领他的研究小组,终于找到了结核病菌中帮助其感染、潜伏人体的关键基因。这一成果于2月19日在线发表在国际学术期刊《基因组研究》上。
《北京科技报》:目前,人类受到结核病菌威胁的情况怎样?
何正国:结核病菌一直伴随着人类的生活,自从发现了卡介苗等一些有效的预防、治疗药物,结核病菌就得到了很好的控制,几乎没有人再因为结核病死去。但是,现在这些预防与治疗手段已经落后了,结核病又有抬头之势,这主要因为卡介苗已经是40多年前的疫苗了,我们至今一直在使用而没有更新。然而,结核病菌一个很大的特点就是潜伏能力很强,潜伏中,它慢慢知道了药物的作用,于是躲开了药物作用,从而产生了抗药性。现在,有4~5亿的中国人都是带菌者,而全球有20多亿带菌者,只是没有发病。
事实上,我国甚至全球对结核病菌的研究都远远不够。
《北京科技报》:很多人并不会像关注乙肝、艾滋病那样去关注结核病。这是为什么?
何正国:这和结核病本身的发病规律有关。以艾滋病为例,一方面它的发病周期比较短,另一方面人们对它的传染途径比较了解,知道怎么预防。而结核病本身发病周期长,95%的带菌者一般不会发病,因此人们往往不太注意。事实上,一旦发病,就会很难治疗。
《北京科技报》:为什么现代医学技术还难以对付“结核病菌”?
何正国:1993年,世界卫生组织就针对结核病宣布“全球进入紧急状态”。由于结核病菌通过几十年的潜伏,以及在与人类的斗争中产生了耐药性,很多治疗手段都将失去疗效。耐药的结核病菌又会在人与人之间、人与动物之间,通过空气非常快地传播开来了。这些散布后的病菌也都是属于耐药性的,没有药物可以治疗。结核病菌一旦爆发就难以预防,因为它是通过空气传播。
《北京科技报》:您发现了结核病菌袭击人体的一个关键基因,它是如何危害人体的?
何正国:这个基因叫做“whiB-3”。结核病菌入侵人体时,会调动一些特定的基因抵抗人体免疫系统。此外,另一些特定基因,还会帮助侵入人体内的结核病菌从人体脂肪中获得能量,以存活下来。而这些特定基因中的绝大部分都是由“whiB-3”控制的。因此,“whiB-3”很可能与结核病菌的感染、潜伏有着很大的关系。
《北京科技报》:这一发现是否会给结核病的治疗带来变革?
何正国:我们的发现只是第一步。如果进一步证实了“whiB-3”的全部功能,我们控制了它,就等于控制了结核病菌的很多功能。那么,针对这个基因研究的药物,就可能控制结核病菌在生物体内的活动,从而对结核病起到治疗效果。
