中医眼中艾滋病病因

肾不藏精艾滋病的易感人群，经十几年来大范围、大标本的研究，已经相当深入细致。动态跟踪的流行病学调查，大致有这些人群：性乱者、吸毒者及血友病人、同性恋及双性恋、多性伴侣者，甚者正常的夫妻、情侣、受辱妇女等有性交行为的人群，如一方有HIV感染，就可能传染给另一方，有一次性接触即传播H1V者。
(一)邪毒外侵艾滋病毒通过性接触、血液接触、母婴传播三种途径进入人体而致命，是一种有高度传染性的疾病。不少中医认为，本病属于温病范畴。中医经典《黄帝内经素问?刺法论》中提到：“五疫之至，皆相染易，无问大小，病状相似”;明代吴有性的《温疫论》是一部论述中医对传染病认识的专著，指出“瘟疫之为病，非风、非寒、非暑、非湿，乃天地间别有一种异气所感”。这里的“异气”，又叫“疠气”。从中医病因学角度，艾滋病似可归于温病中的瘟疫。而本病又有潜伏期长，发病缓慢，但一旦发病后，病情急剧发展，出现高热不退、皮下出血、痴呆昏迷和泄下无度等症状，而致很快死亡。这种情况，正如清代温病学家王士雄在《温热经纬》一书中所述的：“伏气温病，自里出表，乃先从血分而达气分，…不比外感温邪，由卫及气，自营而血”。这里的“卫气营血”是温病发展的四个阶段，也是温病病情的四个层次，一般的传染病由卫分到气分到营分到血分，称为由表达里，病情由轻到重，而伏气温病则一发病就出现血分、营分证候，病情比较危重。艾滋病多数病例的病情与“伏气温病”很相似，是否可以归于此。中医对于艾滋病病因总体认识我们考虑是伏气温病，这不但比较符合艾滋病的发病情况，同时也对艾滋病的急骤多变的临床表现和治疗的艰难复杂，提供中医理论依据。对艾滋病不同期、不同型以及各种机会性感染而出现各异的临床表现，中医将分别予以不同的病因分析。
(二)肾不藏精艾滋病的易感人群，经十几年来大范围、大标本的研究，已经相当深入细致。动态跟踪的流行病学调查，大致有这些人群：性乱者、吸毒者及血友病人、同性恋及双性恋、多性伴侣者，甚者正常的夫妻、情侣、受辱妇女等有性交行为的人群，如一方有HIV感染，就可能传染给另一方，有一次性接触即传播H1V者。当然，其传染概率以性乱者、性行为频发者为高。这种人群从中医理论来分析，凡房事过度、性欲妄动、不正常性生活、嫖娼、频繁性交等皆能耗伤肾精，导致肾精匮乏之“精亏状态”。人体肾精亏损是温疫邪毒容易侵入的内在因素，正如清代温病学家叶桂所说：“冬不藏精，春必病温”。肾精不充，人体的阳气、阴血也生化无源，抗病能力大大下降，血络空虚，不但易感温毒之邪，循血络之虚，乘精室之亏，深潜伏匿于营血之舍，消烁正气，伺机发作;而且容易导致其它病因的侵袭(如风、寒、暑、燥、火等外邪)，机体各脏器功能及抵抗力低下(如肝、心、脾、肺、肾的阴阳气血)，导致各种病机发生发展(如气血两亏、肺气阴虚、脾胃衰弱、肝肾不足等)，这是从机体本身的内因，用中医理论来解释艾滋病证候的多样性、繁复性、迅变性和难治性。从病因角度分析中医对艾滋病的总体认识，如上所归纳即外感邪毒及肾精亏损两方面。至于其它艾滋病高危人群，如吸毒者、血友病者等，是否肾精亏损呢?中医认为凡毒品(如鸦片、吗啡、海洛因、咖啡因之类)都能使人异常兴奋、性欲亢进(暂时)、心神恍惚、飘渺幻想等，是因为毒品之性质燥烈发散，能耗损人体精气，造成肾精匿乏。从临床上观察，毒瘾者大抵面容枯萎、消瘦脱形、性情暴躁、精力衰退、性功能减退、腰酸乏力、头晕目眩，属肾精不足之证。至于血友病，是由于先天性缺乏血液中的凝血因子，中医认为精生血，血液内某些因子缺乏，与肾精不足有关;当然，由于HIV的侵袭，血友病者T淋巴细胞亚群失调，T4细胞下降，提示血友病与艾滋病有类似的获得性免疫缺陷状态，从另一侧面反映了中医的肾精不足。
