谁泄露了艾滋病感染者的秘密
当江妹在沿海某省检测出感染艾滋病以后，为何她另一省的娘家、湖北崇阳的夫家都会在第一时间知晓？甚至在他们还在回家路上的时候，当地疾控部门就已经上门了

11月1日，阿地（化名）登上了从武汉开往北京的火车。在此后几天的一个艾滋病感染者交流会议上，他再一次确认了一个信息:国家不会把他消灭掉。
自2008年6月被确认为艾滋病感染者后，年仅28岁的湖北崇阳人阿地和他刚刚新婚一年的24岁妻子丽丽（化名）一直为此担忧着。
同一天，湖北当地媒体披露了一则消息，称“湖北崇阳县发现73例艾滋病感染者”。经网络转载后，这一消息广为传播。
“我们给害惨了。”湖北省崇阳县委宣传部宣传科科长蔡亚君告诉《瞭望东方周刊》，看到这一消息后，原本辛辛苦苦通过招商引资吸引过来的投资商很多都打起了退堂鼓。
阿地和丽丽夫妇正是“七十三分之二”，他们和崇阳当地的众多百姓、官员一样，时至今日依然被头顶上的“艾滋阴云”笼罩着。
崇阳的病例数在全国平均水平以下
崇阳县地处湖北省东南山区，离武汉有三小时车程。
11月初的报道让它一夜间闻名全国，“艾滋县”的名声不胫而走。
媒体报道称，截至2009年9月底，该县发现73名艾滋病感染者，其中72名是农民，59例是外出务工期间感染。
对这一数字，崇阳某乡村民大兵（化名）并不奇怪。他告诉本刊记者，据他的观察，真正的感染者数字只会比这更多。
崇阳县疾控中心退休干部陈德甫也向本刊记者透露，如果继续加大检测力度，将会有更多的感染者被发现，“其他的县不是没有，是没有去检测。”
湖北省疾控中心一不愿透露姓名的官员向本刊记者介绍，外界对崇阳发现73例艾滋病感染者的解读有误，“当地从发现第一例到现在总共有73例，并不是今年一年就有73例，实际上今年新发现只有十几例。”
该官员进一步表示，崇阳的病例数依然在全国平均水平以下，“并不是很严重”，“当初是根据省里的要求组织记者下去采访的，本来是说崇阳这方面的工作做得很好。”
武汉科技大学教授吴炽煦向本刊记者介绍，感染者“发现得越早越主动”，只要治疗方法得当可以实现长期存活。
然而，不可否认的是，一方面是当地加大检测力度，另一方面却是可疑人群的极力排斥。
大兵的妻子江妹（化名）也在2008年初被证实为艾滋病毒感染者。在此之前，她的工作是“小姐”，从业近十年。前两年发现患妇科病，由于医生的警觉，她被建议速去检测。
从医生的神态中感到异样的江妹选择了回避，她的担心和阿地一样:“听人说谁有这个病要被关起来。”
江妹告诉本刊记者，她被检测出感染艾滋病后，很长一段时间内被当地村民隔绝，“连麻将也没人和我打。”
安全套
在崇阳检测出的73例艾滋病感染者中，除三例是因为吸毒和输血感染外，其余70例都是因为性传播感染。
江妹向本刊记者坦承，她早在1997年就被人诱骗到崇阳做了一名“小姐”，迄今依然和当地的不少“姐妹”保持着很好的关系。
江妹1997年小学毕业后，因弟兄姐妹众多，她随着“打工潮”南下广东，不久认识了一个崇阳男人，谈上了恋爱。此后，江妹就被男友带到了崇阳，被迫成为一名“小姐”。
“那些年主要是自己年轻，一个月赚得多可以拿到两三万。”江妹认为，她应该就是在这个过程中染上艾滋病的。
但是直到最后被确诊感染，江妹也不敢相信，“只知道这病很厉害，得上了就得死。”
她向本刊记者坦承，在和客人交易时，她并不清楚安全套能有效降低艾滋病的感染几率。
华中科技大学同济医学院教授吴均林曾对湖北省多个地方的艾滋病感染情况做过调查，他发现，感染者对哪些防护措施可以有效降低感染风险知之甚少。
一个不容回避的现实是，通过性传播感染艾滋病的比例正在逐年上升。
公开资料显示，截至2009年6月底，湖北省累计报告的艾滋病毒感染者5132例中，经性和注射吸毒传播的比例明显上升。该省2003年发现的感染者中，这个渠道的感染人数只占11.5%，而2009年上半年，同一比例为65.39%。
不过，近些年因为当地疾控部门加大宣传普及力度，有关人员的防艾意识有了显著增强。
陈德甫退休后，负责一个名为“铜鼓博爱小组”的草根组织，推广使用安全套，“我们设有监督员，去旅馆、KTV、娱乐场所宣传。”
在娱乐场所推广百分之百安全套使用项目，正是湖北省的成功经验之一，并曾经向其他省市推广。在武汉市疾控中心就设置有专门的“安全套项目管理办公室”。
湖北省疾控中心某官员向本刊记者表示:“这一措施效果很好，但是因为在某些省推广不够，所以潜在感染的危险很大，而且广泛存在。”
吴炽煦也在调研中发现，目前，湖北“很多地方的小姐的戴套率还是很高的”，这跟有关机构和志愿者的宣传教育有关，“甚至包括跟客人怎样讨价还价，都是培训的内容。”
上述官员也表示，崇阳此次披露的73例艾滋病感染者中，有59例就是在外打工期间感染上的，这说明“有效控制了县内的感染”。
实名制
2008年，江妹最终还是去做了检测，被证实感染艾滋病。当时她正和未婚夫大兵在沿海某省做水果生意。
很快，他们的水果摊就摆不下去了，因为获知消息的顾客都不再上门。大兵无奈，只得把水果摊低价出让，选择回家治疗。
然而没想到，他们还在回崇阳的路上，当地疾控部门的七八个工作人员，已经在村主任的带领下坐在他家等待。
原本想着回家后保密的大兵和江妹顿成了村里的“明星”，男女老少围着他们看新鲜，各种议论随之而起。
这种“明星”般的待遇阿地也曾享受过。他的妻子丽丽在邻近的通城县被检测出来后，还没有回家，当地疾控部门的三辆车就开到了他家。
“我下午两点知道她感染的事，十分钟后20里外的村民都已经知道了。”阿地说，当他也被检测出感染上艾滋病时，他关在家里一个月没敢出门。
阿地拒绝了本刊记者前往他家探望的请求，“你们一去，村里人就都会跑来看热闹。”
江妹也在本刊记者前往她家看望之前一再交代:“别人问就说是我表弟。”“我是艾滋病”的标签早已经压得这位32岁的村妇抬不起头来。迄今为止，她在外省的娘家依然不准她回去。
据吴炽煦介绍，根据有关规定，艾滋病感染者在检测、服药、治疗时都会要求实名登记，感染者的真实姓名、通讯方式、户籍地址等会记录在案，国家相关的法律法规对此的保密也极其严格。
但这正是让江妹不解的地方。当她在沿海某省检测出感染艾滋病以后，为何她另一省的娘家、湖北崇阳的夫家都会在第一时间知晓，甚至在他们还在回家路上的时候，当地疾控部门就已经上门了？
吴均林在调研中发现，与以往卖血感染的患者坦然面对不同，经由性传播感染上的村民对此表现得非常敏感。
“不能讲是疾控部门故意泄露的，农村的实际情况让保密工作变得很难。” 吴炽煦介绍，她在调研中发现过不少因为当地有关部门没留神、弄得满村皆知的例子，最后感染者不得不举家逃离。
生存
作为一个外地人，江妹在崇阳乡下的日子并不好过。除了因为是艾滋病感染者外，31岁的她迄今没有生育也是缘由之一。
大兵和江妹这对朴实的村民一再向本刊记者询问:“我们能不能生孩子？”大兵说，他们为此考虑了很长的时间，但始终不敢决定。
当地疾控部门的劝说让他们犹豫。大兵承认，他们自己生活都存在问题，怎么养活孩子让他们顾虑重重。
2008年8月，已经怀胎7个月的丽丽在当地疾控部门的劝说下把孩子打掉了。丈夫阿地觉得，“我们这里是乡下，有个孩子还是不一样。”
今年阿地和丽丽的孩子终于出生。短暂的喜悦过后，如今已经三个月的婴儿又开始成为阿地心中的“痛”:查还是不查？
阿地一家三口开着农用三轮车来跟本刊记者见面时，随身带着奶粉。婴儿啼哭时，阿地拿出杯子泡好奶粉，搅拌前还特意用开水把勺子烫了一下。
阿地说，专家再三交待他婴儿不能母乳喂养，否则会增大传染几率。
新的问题又来了。每月五六百元的奶粉钱，成了阿地夫妇不堪承受之重，“前几天把结婚时买的3000块钱的液晶电视卖了1000块钱。”
自查出双双感染艾滋病后，阿地夫妇就再没出去打工。丽丽在带孩子的同时已经开始服药，阿地尽管还只是病毒携带者，但原本就身量单薄的他靠苦力赚钱已经越来越不容易。
阿地介绍，在跟本刊记者见面的当天上午，他去帮邻居干活挣了二三十块钱，“把水泥砖从一楼挑到二楼，一角五一块，别人一次能挑四块，我只能挑两块。”
阿地说:“崇阳疾控的人来看我，说我得了这个病要吃好点，可是钱从哪来？”
他听说艾滋病感染者喂养小孩每年有三四千元的奶粉补贴，为此他去找过崇阳县疾控中心，但得到的答复是“现在没有，你先把买奶粉的票留着”。
救助
著名防艾专家、武汉大学中南医院教授桂希恩曾经多次走访阿地、江妹的家，在江妹眼里，“桂教授没有解决不了的问题。”
11月23日，桂希恩教授在接受本刊记者采访时却颇感无奈。曾经有一个感染者找到他，称他想搞副业需要六万元，让桂教授帮他解决，“我也无能为力呀。”
采访当日，桂希恩正忙于审看与香港某基金会的合作项目文书。他表示，他所能做的只是随时帮感染者留意，尽可能争取到更多的资金帮助他们。
在本刊记者走访崇阳艾滋病人时，他们拿出一摞摞医疗费用的单据，问本刊记者哪里可以报销，“我们找过疾控中心，都是说把票收好以后再说。这以后是什么时候？”
据了解，湖北省对感染者的相关疾病治疗报销部分药费，一般家庭每个病人每年报销1000元，特困重症病人每年报销5000元。
此外，从2005年开始，湖北省全面推行包保责任制，对每一个感染者、病人及其家庭，都指定责任单位或动员社会力量实施一对一帮扶。
但这在大兵的眼里显然不够，“我哪一次去医院不是好几百？”
阿地和大兵两人分别都已经背负着近万元欠款，“每次赚到一两百块钱就赶紧先给人家还上，不然下次就不好借了。”大兵说，妻子江妹在发病时还曾多次遭到拒绝收治。
2008年，江妹在服药后出现不良反应，被紧急送往崇阳县人民医院，但医院以床铺已满为由拒绝收治。
大兵说:“有一个医生私下跟我讲，收了她的话，旁边床铺的病人就会吓跑。”后来在崇阳县疾控中心的干预下，由崇阳县中医院收治了江妹。
阿地有一次去当地卫生院抽血，对方见他上门，老远就把门关上了。
希望
阿地曾在上海卖了多年西瓜，掌握了不少西瓜种植的知识。回到崇阳后，他在当地租了三亩地种西瓜，按照设想，一亩地可以赚1000多元，但因为没钱，投入不足，收成很不好。
陈德甫介绍，鼓励感染者生产自救是他们“铜鼓博爱小组”的工作之一，但效果并不都是立竿见影。
“国家有没有可能借给我两三千元本钱？”对类似阿地这样的询问，陈德甫也无能为力，他的小组向有关防艾项目成功申请到的基金也才几千元。
本刊记者就上述疑问采访崇阳县宣传、卫生、疾控等部门时，他们都选择了沉默。蔡亚君对本刊记者坦承:“现在不能再对媒体说什么，崇阳已经戴上艾滋病的帽子了。”
吴炽煦在武汉科技大学牵头成立了一个名为“金色港湾”的感染者组织，不定期召集感染者聚会。她认为，在短时间不能解决他们的实际困难的情况下，给感染者提供心理上的交流、发泄平台非常重要，“每次聚会完，很多病友都手拉着手不想离开。”
